MÉDECIN GÉNÉRALISTE HAD (F/H)
Missions principales :
Le praticien assure la prise en charge optimale des patients à leur domicile :
- Référentiel médical : Être le référent médical de son secteur, évaluer les possibilités de traitement en collaboration avec le médecin hospitalier et le médecin traitant, et apporter son expertise dans la prise en charge de la fin de vie ou de la douleur.
- Coordination des soins : Participer à l'élaboration et à la réactualisation du projet de soins personnalisé, assurer la transmission des objectifs aux différents partenaires, et informer les médecins impliqués des traitements.1
- Relationnel et social : Collaborer avec les services sociaux pour ne pas négliger les aspects familiaux, professionnels et sociaux de l'accompagnement personnalisé.
- Promotion et développement : Participer à la promotion de l'HAD auprès des médecins prescripteurs (en tant qu'expert en douleur, fin de vie, gériatrie) et contribuer aux réflexions sur le développement de prises en charge innovantes (télémédecine, chimiothérapie à domicile, réadaptation, etc.).
Les prises en charge sont variées : soins palliatifs, ERI SP, pansements complexes, et soins techniques (transfusion, surveillance de chimiothérapies, Ferinject). Le candidat recherché est un médecin inscrit à l'Ordre des Médecins en France et titulaire d'un diplôme :
- En médecine générale.
- Ou en médecine d'urgence.
- Ou en Médecine Physique et de Réadaptation (MPR).
Une expérience ou des qualifications spécifiques sont attendues :
- Expérience significative en soins palliatifs et/ou prise en charge de la douleur (DU apprécié).
- Une expérience en HAD serait un atout.
- Le permis B est requis (véhicule de fonction fourni).
