Due to continued business growth, we are seeking an experienced and compassionate Community Worker to join our team on a full-time basis.
Key Responsibilities
The successful applicant will be responsible for:
- Assessing participants' individual needs and assisting in the development and implementation of personalised support plans.
- Providing person-centred support to assist participants in achieving their goals, independence and community inclusion.
- Supporting participants to access community, recreational, educational and employment opportunities.
- Assisting participants to develop independent living, communication and social skills.
- Monitoring participants' progress and maintaining accurate case notes, reports and support documentation.
- Liaising with participants, families, carers, allied health professionals, support coordinators and external service providers to ensure coordinated service delivery.
- Providing information and referrals to appropriate community resources and support services where required.
- Supporting participants with capacity-building activities and encouraging active participation in community life.
- Assisting participants to access transport and attend appointments, social activities and community programs.
- Ensuring all services are delivered in accordance with NDIS Practice Standards, organisational policies, safeguarding requirements and relevant legislation.
- Promoting participant dignity, privacy, choice and cultural diversity in all aspects of service delivery.
These duties align with the role of Community Worker (ANZSCO 411711), which involves assessing client needs, developing and implementing support programs, monitoring client progress, referring clients to appropriate services, assessing community needs and liaising with community organisations to improve access to services.
Essential Criteria
- Diploma or higher qualification in Community Services, Disability, Social Work or a related discipline.
- At least one year of relevant full-time experience in disability or community services (or equivalent combination of qualifications and experience).
- Sound understanding of the NDIS and person-centred support principles.
- Excellent communication and interpersonal skills.
- Ability to prepare reports and maintain accurate participant records.
- Current National Police Check.
- NDIS Worker Screening Clearance.
- Current Working with Children Check (where applicable).
- Current First Aid and CPR Certificate.
- Current unrestricted Australian Driver Licence.
- Ability to obtain a valid International Police Clearance (if required).
Desirable
- Experience supporting participants with intellectual, physical, psychosocial or neurological disabilities.
- Experience working within the NDIS framework and using electronic client management systems.
- Knowledge of positive behaviour support and trauma-informed practice.
