- Accueillir et établir un diagnostic social partagé avec les personnes en difficulté
- Informer sur les droits administratifs, les aides sociales, et les recours possibles
- Construire un plan d'aide personnalisé et accompagner dans les démarches administratives
- Développer des relations avec les partenaires locaux et assurer une veille sociale active
- Collaborer avec l'équipe pluridisciplinaire et participer aux réunions de suivi d'activité
