Annualized salary range: $51,356.00 - $60,091.00 commensurate on skills and experience, plus Group Benefits & HOOPP.

Location: Oshawa with flexibility to work or transition between and of DCHC locations as needed

Position Overview
Reporting to the Manager, Integrated Care & Experience, the Community Health Worker works in partnership with community organizations and service partners/providers as well as community groups and individuals to achieve delivery of health and wellness programs for the community. In this role, the Community Health Worker promotes and assists in the development, implementation and supervision of health and wellness programs.
The Community Health Worker practices the principles of client-centered services within the context of a self-directed and inter-professional team approach. Additionally, this role will collaborate within an inter-professional team of health professionals across the community network that influence the health environment targeted to clients, as well as within the Agency’s team structure, in support of goals that align with and achieve the Agency’s strategy, Mission, Vision and Values.
Key Responsibilities:
- Full supervision of two major programs - After school program and Canada Summer Jobs for summer camps
- Develops, implements and evaluates programs, services and community initiatives in response to community needs in order to achieve organizational objectives.
- Works with families (or individuals) in the community to identify their interests and potential and to help them develop meaningful activities, in line with program/services mandates.
- Ensures all necessary paperwork (e.g., registration forms, permission forms, incident reports, attendance feedback forms, etc.), statistics, reports/evaluations, and other important client information are reported and updated to the manager and relevant client records system.
- Acts as an advocate for clients by providing assistance/links to a variety of resource networks, services and programs in the community as a way of improving their overall health and well-being.
- Provides supervisory duties to variable temporary/part-time, student placements and/or volunteers, on a day to day basis, including recruitment, training and development, and provision of goals and objectives through ongoing coaching, support and direction, as required.
- Works responsibly within assigned program budgets for effective financial and human resources to achieve established goals for the group.
- Completes program proposals and evaluations for the annual work plan to ensure alignment with DCHC’s Vision, Mission, Values and strategic objectives.
- Actively leads/participates on internal cross-collaborative efforts, projects/committees and external healthcare networks as applicable, to influence/keep abreast of trends, best practices and partnerships to advance the Agency’s goals.
- Leads/assists with various community events and group initiatives, both internally and externally.
- Develops, implements, monitors, and evaluate strategies and programs to meet the needs of parents, children and families in the community.
- Liaises and works in collaboration with other community groups, advisory committees and other individuals and organizations supporting parents/families in the area.
- Acts as an advocate for DCHC’s clients by providing assistance/links to a variety of resources in the community.
- Works to develop positive relationships with key stakeholders, other organizations, parents and caregivers.
- Identifies and seeks support to act on opportunities for program and project improvements.
- Other duties, as required for the scope of the role, and as assigned by the Manager.
Administration, Quality Improvement and Reporting
- Document within DCHC’s EMR, information regarding outreach contacts.
- Complete Incident Reports as required.
- Follow policies and standards regarding confidentiality and documentation.
- Ensure all necessary reports are completed accurately and in a timely manner.
- Attend relevant team meetings and planning activities.
- Participate in quality improvement and evaluation activities.
- Work with the health and safety guidelines provided by the agency, which includes annual mandatory training requirements.
Qualifications
- Completion of a formal 2-year post-Secondary program at a Community College (Health/Social Services).
- Three years working with children and youth.
- Progressive supervisory (budget, program, people and performance) experience preferably in health or social service setting, and across an inter-professional team setting.
- Proven research, evaluation, planning and risk management skills.
- Proven conflict resolution and negotiation skills with diverse stakeholders.
- Communication skills, including facilitation, public speaking and presentation skills.
- Demonstrated ability to coordinate and monitor activities of a substantial and diverse range of projects/initiatives which also includes a diverse range of people.
- Proficiency with computer skills and experience (MS Office, database experience).
- Demonstrated organizational and time management skills
Working Conditions – Physical/Sensory/ Mental Demands Analysis:
- Required to be in attendance and involved during some evening and weekend hours, depending on the demands of the particular projects/initiatives.
- Required to work independently, while keeping the goals of the Agency, non-project and other sector of the community in mind.
- Required to travel regularly within Durham, using own vehicle.
- May experience stress and stress related symptoms due to regular interaction with clients in crisis.
- May be exposed to illness and unsanitary conditions in which established H&S protocols must be adhered to at all times. In this, may be in contact with individuals and families in crisis who may be using substances and/or not attentive to personal health and safety for themselves or others.
- May have to manage physically threatening clients and be prepared to be responsible for own safety and safety of others.
- Regular interaction and collaborative work with cross-disciplinary colleagues and community care partners.
- Must monitor their own time and stress levels to ensure that they are able to effectively assist clients and colleagues. They may find that they are continually interrupted, and may have to quickly assess situations to respond appropriately.
- May be placed in life-threatening situations for others such as instances of attempted suicide, violence or other mental-health and physical related risks.
