La Fondation Saphir, fondation multi-missions et multi-sites (psychogériatrie, gériatrie, psychiatrie adulte), est active dans le domaine médico-social dans le canton de Vaud et dans l’accueil et l’accompagnement de personnes fragilisées dans leur intégrité physique et psychique au cours de leur processus de vie.
Nous recherchons pour notre établissement l’EMS Contesse à Croy qui accueille 36 Personnes et qui est spécialisé dans le domaine de la gériatrie :
Assistant·e en soins et santé communautaire (F/H/D) - CDI - entre 60% et 80% - EMS Contesse
Quelle que soit leur fonction, les professionnel·le·s de la Fondation Saphir,ont une volonté commune, celle d’accompagner les Personnes au quotidien pour leur permettre d’expérimenter concrètement la stratégie institutionnelle de prestations « comme à la maison ». C’est-à-dire, conserver leurs habitudes autant que possible, offrir une réponse adaptée à leurs souhaits et, par le biais d’une réelle collaboration interprofessionnelle, permettre le bien-être et le bonheur des Personnes.
En tant qu’assistant-e en soins et santé communautaire, vous serez amené·e à encadrer et accompagner les Personnes dans la vie quotidienne et prendrez part à leur accompagnement dans sa globalité en évaluant leurs besoins physiques, psychiques, sociaux et spirituels. Vous participerez à l’élaboration des projets d’accompagnement et dispenserez des actes médico-délégués sous délégation de l’infirmier·ère diplômé·e et saurez privilégier la collaboration interprofessionnelle. De plus, vous participerez à l’encadrement des nouveaux professionnel·le·s, apprenti·e·s et stagiaires du service accompagnement.
