Au sein d'un établissement médico-social, vous accompagnez des personnes en consommation active vers la réduction des risques et des dommages
- Vous évaluez les besoins de santé et assurez un accueil adapté aux personnes accompagnées
- Vous dispensez des soins liés aux plaies et aux infections selon les protocoles en vigueur
- Vous conduisez des actions de prévention et de réduction des risques liés aux usages de substances
- Vous orientez les personnes vers les dispositifs sanitaires sociaux et médico-sociaux appropriés
- Vous participez à la coordination avec les partenaires et à la traçabilité des accompagnements
Et voici les modalités de l'offre :
- Contrat: CDI
- Salaire: 16.5 euros /an Vous êtes infirmier ou infirmière diplômé(e) d'État, expérimenté(e) et engagé(e) dans la réduction des risques auprès de publics vulnérables
- Vous détenez le Diplôme d'État d'Infirmier et justifiez d'au moins trois années d'expérience professionnelle
- Vous maîtrisez les soins de plaies et la prévention des infections dans un cadre médico-social
- Vous accompagnez les usager(ère)s dans une démarche respectueuse de réduction des risques et des dommages
- Vous faites preuve d'écoute, de non-jugement et d'adaptation auprès de publics en situation de grande précarité
- Vous collaborez efficacement avec les équipes pluridisciplinaires et les partenaires du territoire
