Job Title: 2 x Support Worker
Contract: 1 x full time permanent and 1 x part time
Hours: 38 hours per week & 19 hours respectively
Salary: £29,334 per annum pro rata for part time

About the Role
We are looking for proactive, compassionate and reliable Support Workers to join our dedicated teams in Tower Hamlets & Hackney, London. In this role, you will provide direct, person-centred support to adults with learning disabilities, autism, mental health needs and/or complex support requirements, helping them to live as independently and safely as possible.
As a Support Worker, you will support people with daily living skills, personal goals, community access, health appointments, household tasks and medication support in line with individual support plans. You will also contribute to key working, risk assessments, accurate record keeping and safeguarding practice, while building positive relationships with the people we support, families, colleagues and professionals. You will play an important part in ensuring our services are safe, inclusive, person-centred and aligned with Outward’s values of engage, enable and empower.
Your Responsibilities
As a Support Worker, you will:
- · Provide direct, person-centred support to adults with learning disabilities, autism, mental health needs and/or complex support requirements.
- · Support people to develop daily living skills, maintain their independence, access the community, and work towards personal goals and aspirations.
- · Assist medication support, health appointments, household tasks and other day-to-day needs, in line with individual support plans.
- · Act as a key worker where required, contributing to person-centred support plans, risk assessments, reviews and outcome-focused records.
- · Promote choice, dignity, privacy, safeguarding, inclusion and positive risk-taking in all aspects of support.
- · Build positive relationships with the people we support, families, colleagues, health professionals and other stakeholders.
- · Accurately record and report information, including daily notes, incidents, safeguarding concerns, health updates and changes in support needs.
- · Follow policies and procedures relating to safeguarding, medication, health and safety, infection control, EDI and confidentiality and data protection.
- · Support the safe running of the service by completing health and safety checks and reporting maintenance issues when necessary.
- Work flexibly across services, including evenings, weekends, bank holidays and sleep-in shifts, to support consistent service delivery.
Essential Criteria
- Good understanding of safeguarding and risk management in a care/support setting
- Knowledge of health and safety issues within supported housing or care environments
- Understanding of confidentiality, GDPR, and data protection principles
- Staff must have access to a smartphone capable of receiving SMS messages and installing authentication apps for secure systems access
- Right to Work in the UK. Please note: Outward will not provide visa sponsorship.
Desirable Criteria
- NVQ Level 2 or 3 in Health and Social Care (or equivalent)
- Additional training in PBS, safeguarding, or housing-related legislation
