We are seeking a passionate and experienced Occupational Therapist (OT) to join our community rehabilitation team. This role is ideal for an OT who enjoys the autonomy of managing their own community caseload while being supported by an experienced interdisciplinary team.

This is a flexible, casual opportunity that can be structured as either an employee or independent contractor position, depending on the successful candidate’s preference. Whether you are looking to supplement an existing caseload, build your community practice, transition into community care, or simply enjoy the flexibility of accepting referrals that fit your schedule, we would love to hear from you.

You will work closely with clients and their families in the home, completing comprehensive assessments, developing individualized treatment plans, and recommending equipment and environmental modifications that promote safety, independence, and quality of life. Many of the individuals we support are living with complex or chronic medical conditions and require Alternate Level of Care (ALC) services.

Beyond delivering exceptional clinical care, there will be opportunities to contribute to new service development, Requests for Proposals (RFPs), quality improvement initiatives, and innovative models of care that can help expand Occupational Therapy services across the community.

As our OT program continues to evolve, there will also be mentorship and leadership opportunities for therapists interested in helping shape the future of community Occupational Therapy. This includes opportunities to mentor Occupational Therapy students during community placements, support the development of future clinicians, assist with onboarding new therapists, and contribute to clinical resources and best practices.

What You’ll Do

As an Occupational Therapist with The Care Company, you will have the opportunity to make a meaningful difference in clients’ lives while enjoying the autonomy and flexibility of community practice. Working collaboratively with an interdisciplinary team, you will:

  • Complete comprehensive in-home assessments within the required referral timeframe, including functional assessments, mobility evaluations, falls risk assessments, equipment needs, and ADL/IADL evaluations.
  • Develop individualized, evidence-based treatment plans that empower clients to maximize independence, safety, and quality of life in their own homes.
  • Provide follow-up visits focused on functional goal progression, mobility, environmental modifications, home safety, cognitive and physical strategies, and caregiver education.
  • Recommend, trial, and prescribe assistive devices, adaptive equipment, and home modifications, collaborating with vendors and contractors where required.
  • Support clients and families through education, coaching, and practical strategies that promote confidence, independence, and successful aging in place.
  • Collaborate closely with nurses, physiotherapists, Personal Support Workers, physicians, care coordinators, case managers, and Ontario Health atHome to support seamless care transitions.
  • Coordinate equipment procurement and assist with ordering supplies through Ontario Health atHome vendors.
  • Monitor client outcomes and modify treatment plans as clinically appropriate to support meaningful, measurable progress.
  • Maintain accurate, timely, and clinically sound documentation.
  • Communicate proactively with the office regarding caseload availability, scheduling changes, and urgent referral readiness.
  • Independently manage your caseload and schedule while receiving administrative, scheduling, clinical, and technical support from The Care Company.
  • Deliver compassionate, ethical, and evidence-informed care while upholding The Care Company’s commitment to clinical excellence.

Beyond Clinical Care

This role offers more than a traditional community caseload. As The Care Company continues to expand its Occupational Therapy services, you may also have opportunities to:

  • Help shape and grow our community Occupational Therapy program.
  • Contribute to new service development and Requests for Proposals (RFPs).
  • Participate in quality improvement and innovative models of community care.
  • Mentor Occupational Therapy students during community placements and help develop the next generation of OTs.
  • Support the onboarding and mentorship of future Occupational Therapists joining the organization.
  • Collaborate with leadership on the development of clinical resources, processes, and best practices.
  • Grow into future clinical leadership and mentorship opportunities as our rehabilitation program expands.

Scheduling

  • Flexible, casual scheduling with no fixed weekly schedule.
  • Accept referrals based on your availability and geographic preference.
  • Initial in-person home assessments are generally expected within 48–72 hours of receiving a visit request within your geographic service area.
  • Ability to independently manage scheduling and communicate promptly with the office regarding availability.
  • Central East Region may include Oshawa, Pickering, Peterborough, Haliburton, and surrounding communities.
  • Travel between client homes is required; access to a reliable vehicle is considered an asset.

Qualifications and Experience

  • Registered and in good standing with the College of Occupational Therapists of Ontario (COTO).
  • Experience providing home and community care, particularly with HCCSS and/or Ontario Health atHome clients, is strongly preferred.
  • Community Occupational Therapy experience and/or hospital discharge and transition-planning experience will also be considered.
  • Demonstrated skill in functional assessments, equipment recommendations, safety evaluations, ADL/IADL retraining, and caregiver education in the home environment.
  • Experience with equipment and supply ordering through Ontario Health atHome vendors is an asset.
  • Strong understanding of community discharge planning, falls prevention, mobility supports, and caregiver training.
  • Strong electronic documentation skills.
  • Experience coordinating with interdisciplinary teams and communicating with case managers.
  • Ability to work independently in the community and produce clear, timely, clinically appropriate documentation.
  • Valid driver’s licence and access to reliable transportation, or the ability to travel efficiently within the designated service area.
  • ADP Authorizer status is considered an asset; however, The Care Company is open to supporting successful applicants through the ADP authorization process.

Compensation

  • $150 per initial assessment, including supply ordering and funding applications.
  • $75 per in-person follow-up visit.
  • $50 per virtual follow-up visit.
  • $25 per 30 minutes of approved administrative time completed outside of scheduled client visits, including documentation, care coordination, report writing, supply ordering, funding applications, and other client-related administrative duties.
